LIST OF SOCIAL SERVICES PROVIDED BY THE BUDGETARY INSTITUTION
VALAKAMPIŲ SOCIALINIŲ PASLAUGŲ NAMAI

I. GENERAL SOCIAL SERVICES

1. Information:
1.1. providing information and consultations to residents of the City of Vilnius on matters related to the social care and support of persons with intellectual, psychosocial and multiple disabilities;
1.2. providing information to individuals and their relatives about the social services provided by the Budgetary Institution Valakampiai Social Services Home (hereinafter – the Institution), as well as the social assistance and support provided by Vilnius City Municipality;
1.3. publishing information about the services and activities provided by the Institution on the website www.valakampiuspn.lt;
1.4. familiarising individuals and their relatives with the internal rules for residents and other documents regulating life at the Institution;
1.5. ensuring that individuals have access to the necessary information.
2. Consultation:
2.1. analysing problematic situations together with the individual (and/or their relatives) in order to identify effective solutions within the scope of professional competence;
2.2. providing preventive and educational consultations;
2.3. enabling individuals to consult specialists from the Budgetary Institution Valakampiai Social Services Home and other institutions.
3. Mediation and representation:
3.1. representing individuals in resolving various personal matters (legal, health-related, financial, household, social, etc.) when, by court decision, the Budgetary Institution Valakampiai Social Services Home has been appointed as the individual's guardian and/or property administrator;
3.2. mediating on behalf of individuals in the institutions whose services they require;
3.3. assisting individuals in handling personal matters outside the Institution;

3.4. mediation and representation in protecting the individual’s legitimate interests and resolving various personal and property administration matters with the relevant institutions.

II. LONG-TERM (SHORT-TERM) SOCIAL CARE

4. Accommodation:
4.1. an environment adapted to ensure the individual’s physical and psychological safety;
4.2. single, double or triple rooms;
4.3. basic furniture and soft furnishings (bedding, etc.);
4.4. the possibility for individuals to have their own furniture and other personal belongings in their rooms.
5. Psychological and psychotherapeutic support:
5.1. consultations with a psychologist at the Institution;
5.2. enabling individuals to receive assistance from a psychologist/psychotherapist at healthcare institutions.
6. Development, maintenance and/or restoration of social skills:
6.1. maintaining and restoring relationships with family members, relatives, friends and acquaintances;
6.2. assistance in maintaining links with the community or participating in community activities;
6.3. strengthening communication skills through participation in community life;
6.4. assistance in creating, restoring or maintaining a network of social relationships.
7. Development, maintenance and/or restoration of daily living skills is implemented for each individual in accordance with their Individual Social Care Plan (self-care, personal hygiene, healthy lifestyle skills, household chores, home safety, maintaining cleanliness in the kitchen and home, food preparation, budgeting, saving and managing money, using banking services, shopping, orientation in the environment, using public transport, etc.), including managing personal finances, purchasing goods and paying bills, planning and carrying out household chores, communicating, independently maintaining living premises and their surroundings, etc.

8. Development of work skills and daytime activities:
8.1. organization of daytime activities at day centres or at the Institution (sewing, knitting, arts and crafts, etc.);
8.2. development of work skills.
9. Organization of leisure activities:
9.1. outings;
9.2. concerts, exhibitions, afternoon events and meetings;
9.3. celebration of birthdays, national and religious holidays;
9.4. walks;
9.5. providing opportunities to read books, newspapers and magazines, watch television and listen to the radio (recreation areas);
9.6. organization of relaxation activities.
10. Assistance with dressing, eating, washing and other types of assistance, organization of personal
hygiene services (laundry, ironing, maintenance and cleaning of living and common areas, etc.).

11. Meals:
11.1. individuals attending the day centre are provided with meals 3 times a day;
11.2. individuals staying at the Institution are provided with meals 4 times a day;
11.3. dietary meals and food preparation in a modified texture are arranged according to individual needs and medical recommendations;
11.4. individuals are provided with the opportunity to have coffee/tea or prepare their own food at any time;
11.5. individuals are provided with opportunities to review the menu in an understandable format and make suggestions regarding the range of dishes.
12. Organization or provision of healthcare (nursing) services:
12.1. organization of medical examinations, prescribed treatment, medical consultations and inpatient treatment at healthcare institutions;
12.2. individual nursing care according to the person’s health condition;
12.3. provision of medication, assistive devices and nursing supplies in accordance with the established standards;
12.4. provision of first aid and accompaniment to healthcare institutions;
12.5. organization of a preventive health examination once a year;
12.6. continuity of prescribed medication treatment at the Institution;
12.7. organization of rehabilitation services at healthcare institutions;
12.8. organization of participation in preventive healthcare programmes at healthcare institutions;
12.9. organization and implementation of preventive programmes at the Institution;
12.10. education of residents on health promotion and health maintenance.

13. Organization of transport services:
13.1. transportation of the individual to the day activity centre;
13.2. transportation of the individual to healthcare institutions;
13.3. transportation to excursions, cultural events, etc.;
13.4. transportation of the individual to other necessary institutions, etc.
14. Other services required according to the individual’s needs:
14.1. development of social responsibility;
14.2. providing opportunities to use means of communication (Internet, telephone);
14.3. provision of footwear and clothing in accordance with established standards.

III. ACCOMMODATION IN AN INDEPENDENT LIVING HOME

15. Accommodation:
15.1. a home environment adapted to ensure the physical and psychological safety of the individual (family);
15.2. one-, two- or three-person apartments with an integrated kitchen and personal hygiene facilities;
15.3. basic furniture, household appliances and soft furnishings (bedding, etc.);
15.4. the possibility for individuals to have their own furniture and other personal belongings in their rooms;
15.5. access to a shared laundry room and recreation room.

16. Development, maintenance and/or restoration of social skills:
16.1. development, maintenance and strengthening of communication skills (seeking assistance, adapting to new situations and environments, participating in community life, establishing and maintaining relationships with relatives, etc.);
16.2. development and strengthening of self-advocacy skills.

17. Development and/or maintenance and/or restoration of daily living skills (self-care, personal hygiene, healthy lifestyle skills, household chores, home safety, maintaining cleanliness in the kitchen and home, food preparation, budgeting, saving and managing money, using banking services, shopping, orientation in the environment, using public transport, strengthening childcare and parenting skills, etc.).
18. Services provided by a healthcare professional at the Institution and/or assistance
in accessing a healthcare institution.
19. Consultations with a psychologist at the Institution.
20. Assistance in finding and maintaining employment.
21. Other services required according to the individual’s needs.

IV. DEVELOPMENT, MAINTENANCE AND/OR RESTORATION OF SOCIAL SKILLS

22. Communication aimed at strengthening communication skills (seeking assistance, adapting to new situations, participating in community life, establishing and maintaining relationships with relatives, etc.).

23. Daytime activities organized through art, handicrafts, computer literacy, film therapy and bibliographic activities, as well as culinary afternoons.

24. Sociocultural services (music and sports activities, various events, outings, etc.).

25. Development of work skills (sewing, knitting, crocheting, embroidery, maintenance of premises and surroundings, development and maintenance of gardening and horticultural skills, etc.).

26. Development and maintenance of daily living skills (assistance with financial planning, planning purchases, paying bills using online banking, planning household chores, learning to prepare meals, caring for children, etc.).

27. Development of social responsibility.

28. Individual and/or group consultations with a psychologist.

29. Decision-making support and other services provided at the individual’s home when, by court decision, the Budgetary Institution Valakampiai Social Services Home has been appointed as the individual’s guardian and/or property administrator.
